Short answer
The quest journal leads to one finale, The Last Battle, and community quest data shows no separate alternate ending. To reach it you must finish all 41 main story and 51 character quests (the 18 side quests are optional), then get 90 reputation with Jack, Gunter, the Nun, Herbert, Albert and Linda. The one choice that changes the road is the Red Eye: keep it and those requests rise to 100 each, plus three extra items.

What the ending requires
The journal holds 110 quests: 41 main story, 51 character and 18 side quests.
The main and character quests are all required. Side quests are optional.
Before the final battle, six characters each ask for 90 reputation: Jack, Gunter, the Nun, Herbert, Albert and Linda. Their character quests pay that reputation 10 at a time, which is why every one of them matters.
The Red Eye choice
In the main quest Friend or Foe you obtain the Red Eye, open the walled-off cheese cellar in the Sewers and meet the Old God. Then you choose whether to give him the eye or keep it.
Give it: the follow-up asks you to hold a special sermon for the Old God in your Church.
Keep it: you must build an attic at home, and the six final requests rise to 100 reputation each, plus an Elegant Prosthetic Hand, a Comfortable Chair and Luxury Drapes.

The road to the finale
Eye, Red Eye opens Gluttony and Pride, Wrath and Sloth, Lust and Envy, and The Last Battle.
The Last Battle also needs Palace Politics finished.
The Last Battle
The Old God gives the final quest: speak with the guards at the post, win the Final Battle, then restore the portal at the Astrologer's Tower with the Greed portal fragment.
Finishing it unlocks The End of the Journey ("You completed the game") and The Other Keeper ("You found your other you").
The journal's closing line reads "The end?" — we have found no source that documents a different ending screen.
