Short answer

The Red Eye is a quest item from the main quest Friend or Foe. You use it to break into Mauritzio's walled-off cheese cellar in the Sewers, where the Old God waits, and then decide to give him the eye or keep it. Giving it only asks for a special sermon in your Church. Keeping it means building an attic (16 Wooden Planks, 8 Iron Details, 6 Carved Stones) and raises the six final reputation requests from 90 to 100.

Getting there

Friend or Foe needs two main quests finished first: Finding Old God and High Price.

Finding Old God comes from Davy the Dagger after Goddess in the Swamp. The looters lead you to Mauritzio's cheese cellar in the Sewers — but the cellar has no door.

High Price sends you to the Trademaster on the Main Pier. His ship only arrives on Lust day, and the third portal fragment costs 1 gold coin.

Friend or Foe

Dig, the mysterious visitor, gives the quest. Keep restoring the portal and rest at home now and then until it moves on.

Obtain the Red Eye and use it on the blocked wall into the cheese cellar.

Inside you find the Old God, unlock The Ancient One achievement, and make the choice.

If you give it

The next quest, Eye, Red Eye, asks you to hold a special sermon in your Church dedicated to the Old God.

The six final requests stay at 90 reputation each.

If you keep it

Eye, Red Eye asks you to build an attic in your home: 16 Wooden Planks, 8 Iron Details and 6 Carved Stones. The Prayer for Old God is provided.

The six final requests — Jack, Gunter, the Nun, Herbert, Albert and Linda — rise to 100 reputation each.

You are also asked for an Elegant Prosthetic Hand, a Comfortable Chair and Luxury Drapes.

Which should you pick?

Both choices lead into the same quests: Gluttony and Pride, Wrath and Sloth, Lust and Envy, and The Last Battle.

Giving the eye is the shorter road, because it avoids the extra 10 reputation per character and the three extra items. Keep it only if you want the longer version of the endgame.
