Short answer
A grave scores the lower of two numbers: its headstone and fence wreaths minus the body's red skulls, and the body's white skulls. So a marble grave over a poor body still scores almost nothing. Match decorations to your bodies, fill spare modules with benches, lanterns and flower beds, and clear stones, stumps and bushes — each one takes points off.

How a grave is scored
Graveyard quality is the sum of every grave's score, plus decorations, upgrades and modules, minus debris.
One grave scores the lower of (headstone wreaths + fence wreaths − red skulls) and the corpse's white skulls.
Red skulls cancel wreaths first. A body with 5 red skulls under 2 wreaths of decoration scores −3.
Headstones and fences
Headstones: Wooden Sign 1 wreath, Wooden Cross I–II 2–3, Gravestone I–II 4–5, Marble Cross I–II 6–7.
Fences: Wooden Fence I–II 1–2, Stone Fence I–II 3–4, Marble Fence I–II 5–7.
Spend marble on bodies with many white skulls. On a poor body, the white-skull cap makes the extra wreaths worthless.
The Anatomy perk Monument builder gives +4 Building mastery when you craft gravestones and grave fences.
Modules: plan before you build
Graves are marked at the graveyard's Blueprint Desk, then dug with a shovel, filled and marked with a sign.
The graveyard is built on modules: place the module first, then the grave or decoration on it. The number of modules is limited, so decide what each one holds before you build.
Use the modules that cannot hold a good grave for decorations rather than leaving them empty.
Decorations
Benches +3 to +5, Monuments +2 to +6, Flower beds +2 to +6, Lanterns +2 to +5.
A Columbarium adds +12 to +20 — the biggest single boost once you can build one.

Clear the debris
Everything left standing in the graveyard costs quality: stones and ore −2 to −3, bushes −1 to −2, trees −1 to −3 and stumps −2 each.
Clearing them is free quality, so do it before you build anything.
Why quality pays
A successful sermon pays 20 to 30 bronze per point of graveyard quality. Prayer for Storms and Money Prayer III pay 30, so 100 quality is 30 silver per sermon.
Reaching 200 quality unlocks the Home Sweet Tomb achievement.
